By selecting continue, and applying for membership cardholder/member agrees to Terms of Use below and authorizes Florida Chiropractic Society to process payment pertaining to membership and in accordance with Cardholder Agreement.
All memberships must maintain a minimum of 7 months consecutive payments to redeem a 20 hour conference at no extra cost. Any membership canceled or not processed for payment prior the 7 months, will be charged a conference full fee.

Two Levels, V.I.P. & FOUNDERS, Include One 20 Hour C.E. Convention Per Membership Year
This is a support level membership. We thank you for your support to the FCS. If you attend CE conferences for credits to any other state besides FL, you will receive a discount and will be billed and automatically charged to the credit card on file. NO FL DC's may hold this level of membership.
Terms of Use:
All levels of FCS membership are by membership year. All “Monthly” members agree to 12 consecutive monthly payments of equal amounts per month according to their level chosen and as explained above. It is the member’s responsibility that when knowingly changing their current credit on file or when receiving notice of a “recurring payment failure” with their credit card on file, to notify the FCS within 24 hours. All levels of membership will automatically renew with the credit card on file into the next membership month or year unless the FCS is notified of a cancellation of membership no later than 30 days in advance of the end of the present membership month or year.
Providing the information contained in this application constitutes your permission for the FCS to contact you regarding related information via mail, email and phone. Your contact information is neither nor sold to anyone and is solely used by the FCS.
IRC Section 6033 (e) Proxy Tax on Lobbying and Political Activities:
The Florida Chiropractic Society is a 501(c)(6) organization. For income tax purposes, member dues paid to the FCS are deductible as a business expense. However, the FCS estimates that 50% of all dues paid to the FCS are utilized for non-deductible lobbying expenditures.
All members are advised that this percentage of the annual dues paid to the FCS is non-deductible for income tax purposes. Consult your tax adviser.

In order for your application to be processed you must submit payment of your registration fee. Memberships are automatically renewed unless cancelled and run annualy from the date of application.
Upon submission, you will be asked to provide credit card information.
